For clearance, please follow steps 1 and 2 immediately. 1. Make an appointment for a physical NOW so that you are able to try out. Be sure to bring this form with you. Return it to Lelsey Franco in the health office or download it to sportsnet. 2. Make sure that your parents/guardians register on SportsNet. Link
